LIV3 Health Father's Day Giveaway, Official Rules

1. Sponsor. LIV3 Health is the sole sponsor and administrator of this giveaway ("Sponsor"). Questions: support@liv3health.com, (833) 407-0747. DEXAfit is not a sponsor of, and is not affiliated with, this giveaway; DEXAfit is named only to describe the scan prize, which Sponsor provides.

2. Eligibility. Open to legal residents of the fifty United States and the District of Columbia who are 18 years of age or older, or the age of majority in their state of residence, whichever is greater, at the time of entry. To be eligible, an entrant must reside within one hundred (100) miles of a participating DEXAfit location. Employees, officers, and directors of Sponsor, its affiliates, subsidiaries, and agencies, and the immediate family and household members of each, are not eligible. Void where prohibited or restricted by law.

3. Entry Period. The giveaway begins June 7, 2026 at 12:00 AM ET and ends June 20, 2026 at 11:59 PM ET (the "Entry Period"). Sponsor's computer is the official timekeeper. Entries submitted before or after the Entry Period are void.

4. How to Enter. There are two ways to enter, each worth one (1) entry, and each with equal chances of winning. (a) Email subscription: complete the subscribe form on this page with your first name and email address to join the LIV3 Health email list. (b) Alternate entry, no subscription: complete the alternate entry form on this page with your first name and email address. The alternate method does not subscribe you to any marketing list and does not include the welcome discount code, which is provided only to subscribers. Limit one (1) entry per person and per email address regardless of method; if duplicate entries are received, only the first is counted. No purchase, payment, or subscription is necessary to enter or win, and subscribing will not improve your chances of winning. Entries that are incomplete, illegible, or generated by automated means are void.

5. Prize and Approximate Retail Value (ARV). One (1) winner will receive: two (2) DEXAfit body composition scans (ARV $400 USD) and a one-year supply of SugarShield by LIV3 Health, twelve (12) bottles (ARV $600 USD). Total ARV: approximately $1,000 USD. The prize is non-transferable, and no cash or other substitution is permitted, except that Sponsor reserves the right to substitute a prize of equal or greater value if the advertised prize becomes unavailable. DEXAfit scans are subject to participating-location availability and must be redeemed within the validity window stated at the time of award. All taxes and any expenses not specified here are the sole responsibility of the winner.

6. Winner Selection and Odds. One (1) potential winner will be selected in a random drawing from among all eligible entries received during the Entry Period. The odds of winning depend on the total number of eligible entries received. The drawing will be conducted on or about June 21, 2026.

7. Winner Notification. The potential winner will be notified personally by email on or about June 21, 2026, at the address provided at entry. If a potential winner cannot be contacted, does not respond within seven (7) days of notification, declines the prize, or is found ineligible, the prize may be forfeited and an alternate winner selected at random from the remaining eligible entries.

8. Taxes. The prize is treated as income to the winner, who is solely responsible for all federal, state, and local taxes on the prize. Because the total approximate retail value is $600 or more, the winner will be required to complete and return an IRS Form W-9, providing a valid Social Security number or taxpayer identification number, before the prize is awarded, and Sponsor will issue an IRS Form 1099 reflecting the value of the prize. Failure to return a completed W-9 within the time specified by Sponsor may result in forfeiture of the prize and selection of an alternate winner.

9. Privacy. Information you provide is collected by Sponsor and used in accordance with the LIV3 Health Privacy Policy, available at liv3health.com/policies/privacy-policy. If you enter by email subscription, you agree to receive marketing email from LIV3 Health and to be added to its email list; you may unsubscribe at any time using the link in any email. If you enter by the alternate method, your information is used only to administer the giveaway and to contact you if you win, and you are not added to any marketing list. Sponsor does not sell your personal information.

10. Limitation of Liability. By entering, each entrant agrees to release, indemnify, and hold harmless Sponsor and its parent companies, affiliates, subsidiaries, officers, directors, employees, and agents from and against any and all claims, liabilities, injuries, losses, or damages of any kind arising out of or in connection with the giveaway, participation in the giveaway, or the acceptance, possession, use, or misuse of the prize. Sponsor is not responsible for lost, late, misdirected, incomplete, or undeliverable entries, or for any technical or network malfunction, error, or failure of any kind. The prize is awarded "as is" with no warranty of any kind, express or implied, to the extent permitted by law.

11. Void Where Prohibited. This giveaway is void where prohibited or restricted by law. All applicable federal, state, and local laws and regulations apply.

12. No Platform Affiliation. This giveaway is in no way sponsored, endorsed, administered by, or associated with Instagram, Facebook, or any other social media platform.
